| Visa Information All private passport holders who are applying for a U.S. visa for the first time or who are applying for reconsideration of a previous application are required to contact the Visa Information Call Center to obtain visa information and make an interview appointment. People calling from within China may dial toll free number 4008-872-333 or Shanghai local phone (021) 3881-4611, while those calling from overseas should dial (86-21) 3881-4611. The Visa Information Center service is staffed Monday through Friday from 7:00am to 7:00pm and Saturday 8:00 am to 5:00 pm, closed on Chinese holidays. Nonimmigrant Visa Application Forms DS-156 Visa Application Form — All applicants applying for a visa with a Chinese passport must complete two DS-156 application forms, one in English and one in Chinese. Each form is two pages, and must be printed on the front and back of each sheet of paper. Only A-4 or 8.5 x 11 inch paper will be accepted. DS-157 Supplemental Visa Application Form — All applicants 16 years of age and older must complete two DS-157 application forms, one in English and one in Chinese. Each form is one page, and must be printed on separate sheets of paper. Only A-4 or 8.5 x 11 inch paper will be accepted.Please note that while filling in Item No. 3 (Full Name in Native Alphabet) of the DS-157 English version, the applicant should write down his/her full name in Chinese characters. DS-158 Contact Information and Work History for Nonimmigrant Visa Applicant — All applicants applying for F (Student), J (Exchange Visitor) and M (Non-academic Student) visas, and their dependents (F-2, J-2 and M-2 visas), must complete two DS-158 application forms, on in English and one in Chinese. Each form is two pages, and must be printed on the front and back of each sheet of paper. Only A-4 or 8.5 x 11 inch paper will be accepted. Visa Application Fee All applicants for nonimmigrant visas must pay a non-refundable application fee of RMB 830 payable at designated branches of CITIC Bank. Both copies of the CITIC Bank fee receipt must be included with all visa applications. Please note that the application fee is non-refundable regardless of whether or not a visa is issued. Applicants who have paid the application fee but fail to submit applications within the expiration date of the receipt for the application fee will not have their application fee refunded.
