Visa Requirements: One (1) completed and signed application form is required. Your signature must be an original -- photocopied or faxed signatures are not accepted. One recent photograph, 2" x 2", color or black and white on a white background, is required. No instant (Polaroid) photos will be accepted. Two letters are required. One letter is needed from the applicant's U.S. company, explaining the precise purpose and description of the applicant's assignment in Brazil and stating its financial responsibility for the applicant, the job position and job description of the applicant in the U.S.A., the person who the applicant will contact in Brazil and his/her job position and job description. This letter must be signed by a company official. Another invitation letter is needed from the company the individual is visiting Brazil. This letter needs to explain the purpose of the trip to Brazil, must be signed and can be faxed. The Visa fee of US$170.00 per passport is required by the Consulate of Brazil (company check, cash or Post Office-only money order; the Consulate does not accept personal checks). A service fee of US$60.00 per passport is also required by Brazil Visa Service (cash, company check or money order only--no personal checks are acceptable). One (1) recent photo, 2" x 2", in color or black and white, (full face only) on a White or very Clear background, taken by a professional passport photographer -- no instant (Polaroid) photos will be accepted. Photocopy of your Round-trip Ticket -- The Consulate also will accept in leiu of a photocopied ticket a signed letter from the carrier or travel agent, on their letterhead, with the name of the passenger, flight number, name of the airline, itinerary and dates of Arrival and Departure from Brazil. One letter per person is required for applicants not presenting a photocopy of their round-trip ticket. If you won't be staying at a hotel, the Consulate requests a notarized invitation letter from the host in Brazil stating citizenship or type of visa, job occupation and address, explanation of how and when they first met the applicant and detailed description of planned activities while in Brazil. For children bretween the ages of 3 months and 6 years, a Certificate of Vaccination against polio must be presented, indicating the date and dosage of the vaccine. It must be stamped by the local Board of Health. For minors (18 years old or younger) the signature of both parents (if applicable) is required on the application form and a letter to the Consulate of Brazil giving authorization for the minor to travel. A Public Notary needs to notarize the parents' signatures. A Visa Fee of $110.00 per passport is required by the Consulate of Brazil (company check, cash or Post Office-only money order; the Consulate does not accept personal checks). All applicants over 16 years of age must present a signed, good-conduct police report (records check). This letter must originate from the Main Police Department from the city from which the applicant has resided for the past year. The report is valid for only 90 days. Applicants under 18 years of age must provide parents or legal guardians authorization to travel to and from Brazil. It will be acceptable as proof of authorization to have the application forms signed by both parents or legal guardians or a letter of consent authorizing the minor to travel to and from Brazil. In any case, the signatures must be notarized by a notary public. An enrollment statement from the school to be attended in Brazil is also required.
