SEAMAN / SEAFARER VISA: Personal appearance; Duly filled up Visa Application Form; One (1) recent photograph, with white or light-colored background; Applicant's national passport valid for at least six (6) months or more from date of departure from the Philippines or completion of trip; If the applicant is a seaman joining ship, the company concerned should provide a letter indicating the name of the vessel he will be joining in the Philippines; Visa fee (to be paid in cash).
